It is as if they want to please the researchers, by ‘doing the right thing’. Of course, this is all supposition, since, quite a lot of the time, they don’t really know what the researchers are investigating. There are a number of other mechanisms through which the placebo effect might work. Some or all of these explanations may be true in any given patient at any given time. First, there is the effect of conditioning. When a person is used to something happening, a special cause–effect bond is created.
Jackson and Lindsay (1995) made significant reductions in dental patients’ anxiety prior to their consultation by giving them a leaflet which explained that the dentist would be willing to stop the procedure if they signalled to them that they did not wish it to continue, either because they were uncomfortable, worried or in pain. On the negative side, however, the positive effects of written communication are not universal. Ley (1997) makes the assertion that up to a quarter of medical leaflets would be understood by as few as a third of the people they are given to.
Because a drug seems to be doing nothing doesn’t mean that this is the case. Just because a person develops an unpleasant symptom after taking a drug doesn’t mean that the drug is responsible for it. Second, drugs can sometimes take a while to start working. People often expect immediate or quick effects, and many drugs do not work in this way. People also mistakenly believe that when a problem appears to have cleared up, then it has gone for good. It is common for people taking antibiotics to stop when the symptoms have gone away, or for people using anti-fungal creams for foot infections, like tinea pedis, to do the same.
